For most of the services we provide, proof of income is needed.
Here is a list of what we typically need:
- Most recent tax return (if you file).
- Current years Social Security Benefits Statement (if you receive benefits).
- Social Security numbers and Dates of Birth for all household members.
- Two of your most recent bank statements.
When you call to set up an appointment, staff will make sure to tell you what you need to bring with you. If you are not sure how to access needed documents, we can help you with that as well.
